In this section, the witness provides personal details needed to identify them, including, their name, age, and occupation. This short bio-profile of a witness allows courts to contextualize their understanding of a testimony. For example, if the witness is a senior citizen or a minor, the validity of their statement may be considered more compromised on account of their age status compared to a healthy and sane 30 year old.

First up, we use a standardized format to pinpoint the exact event or incident being referred to. We add separate sections to identify the exact Date and Time of the incident, as well as its Location (which may be in the form of a specific address or merely allude to a general description of where it occurred, say, next to the community park).
