Managing so many employees who need to be away from work for different reasons can be challenging for any company! Especially if your HR department still relies on paper-based leave forms. Luckily, there is a solution!
Our Time Off Request Form is a print-ready, fillable document that allows you to make a standard form for employees to formally ask for an approved, scheduled leave from work. Doing so helps both the employees and employers keep track of leave requests as well as manage employee absences by ensuring adequate staffing and minimizing disruption to operations.
Our template is simple. It asks for an employee’s details, dates or hours of leave/return, the reason for taking time off, followed by details of persons processing & approving the leave.
Now, let’s go over each detail of our form:
Employee Details
In this part, we simply seek to identify who is asking for time off.
This serves as a record of both, their position within the organization (such as their designation and the department they work for), and their contact details (including Email ID, contact number, and postal address) which may be needed for any future clarification or follow-up.
Since many organizations also assign their employees an Employee ID – a unique identification number, adding this subsection ensures that leaves are correctly tied to the company’s internal system. Moreover, adding the name of a supervisor (if applicable) makes it easier for the company’s senior management to ensure that the right steps are being taken at the managerial or departmental level to ensure the smooth flow of company operations during the employee’s absence.
Request Duration
Here, the employee asking for time off must specify the exact dates (in case of full-day leaves), or the exact hours for which the request is being generated.
Having a precise idea of when the employee will be away from work allows HR managers and other department seniors enough time to manage their absence beforehand, say, by rescheduling shifts to ensure adequate staffing on the dates or hours for which time off is being requested.
Reason for Request
This section is the most important part of the template as it is the justificatory basis upon which you are requesting time off. In our template, we have added separate boxes for the most common reasons (or types of leaves) employees usually request absence from work:
- Vacation
- Family reasons
- Personal leave
- Medical leave
- Funeral/bereavement
- Jury duty
In case your reason for requesting a leave is not covered by any of the categories above, we have added an option to select ‘Other’ so you can specify your reason for requesting time off.
Note however that different organizations may have different policies when it comes to leaves. For instance, employees who are still in their probation period may not be entitled to the same number of personal or vacation leaves as permanent employees.
HoD Approval
In this section, a request for time off is formally approved or rejected. This is usually done by the relevant Head of Department.
For instance, if a salesperson is requesting time off for vacation, the request form will be forwarded to the Sales Manager who must approve or reject the request and add their dated signature to verify this.
For Office Use Only
Unlike the rest of the form which is filled out by the individual employee requesting time off – this section is only intended for use by the HR department.
First up, we have three sections where the HR official keeps a record of the total number of vacation leaves available to the employee in question, the number of leaves requested, and the number of leaves remaining.
This helps an organization track an employee’s leave record which may be used to calculate employee compensation, such as allowances, benefits, and payments. They are also used to make sure that employees are aware of their leave days for future reference.
At the end, a separate section entitled ‘Comment’ has been added where the HR official processing a time off request can make note of anything of importance, such as any time-sensitive tasks an employee must complete before or during their time off.
