Looking forward to rejoining the workforce after a long illness or injury? Exciting as that sounds – before you go back to work, your employer might ask you to submit a formal note endorsed by a doctor confirming you have recovered from your condition, and if so, under what conditions.
That’s when you will need a pre-made Return to Work Note like this, which answers two crucial questions: (a) when can you safely return to work without compromising on your own recovery or the well-being of others, and (b) what precautions or limitations in the workplace (if any) do you need to bring to the attention of your employer.

Patient Details
In this first part of the Note, you add basic details about the previously sick or injured employee, such as their name and age. This is followed by the date and time when the employee was admitted into medical care, and discharged from it.
Statement of Recovery
Our Note starts with a statement of recovery. This is the main point of a Return to Work note, which is why it must be clearly and explicitly stated in the very beginning, as we have done in our template.
In the first blank space, the doctor shall write down their name followed by a second blank space where they must identify the duration for which the employee has been under their medical supervision. This duration may or may not be the same as the Admit Date identified above – for example, in the case of a family doctor or GP, the patient will be known to the doctor much before they were admitted for this specific medical condition.
In the third blank space, the doctor shall indicate a specific date whenceforth an employee will be able to return to work, and discharge their usual professional duties.
Precautions/Limitations
In this section, the doctor shall identify any limitations or precautions that might be needed at the workplace in light of the employee’s health condition.
For example, there may be a restriction on the number of working hours a day they can work or limitations on strenuous physical activities such as operating heavy machinery. In case of certain respiratory infections, the returning worker may need a dust-free environment to avoid triggering their condition.
These limitations and the practical measures needed to overcome them, should be clearly stated in this section, and if there are none, then this should also be clearly stated.
If need be, your employer might ask you for additional medical records (such as laboratory results or test reports). But to do this, they must have a legitimate business reason, and either way, must always abide by privacy laws and only request for information that is relevant.
In the placeholders below, mention the name of your doctor, and their educational or professional qualifications. You also need to add their contact details (E-mail ID and phone number) to make sure an employer can get in touch with them if any follow-ups or clarifications are needed.
Doctor’s Verification
To prove the credibility of this medical document, add a dated signature from your doctor explicitly verifying that the information included in the Note is true and accurate to the best of their professional knowledge and expertise.
Don’t forget to ask your doctor to put their professional stamp on the Note, which shows their practice registration number. This ensures that any information provided in the Note can be traced back to a responsible & registered healthcare provider.
