Feeling a little under the weather, and simply can’t go through another day of grind? Typical flu symptoms like fever, sore throat, or a runny nose can often leave you incapable of being active or productive.

But here’s the thing: whether you are a student missing out on lectures or a professional taking a day off, rules are rules. Before you call in sick, you will be asked to furnish medical evidence of your condition – this, of course, depends on your company or institution’s leave policy.

Patient Details

After mentioning the date and time of a medical check-up, in this preliminary part of the Note, you will add basic details about the patient, such as their name and age. 

Condition

In this section, your doctor or GP will briefly explain your condition, including a formal diagnosis (say, which type of flu virus you are infected with) as well as any specific symptoms you are developing or experiencing, such as high body temperature, coughing, or muscle pain. This section can also be used to identify any side effects of anti-flu medication that might negatively affect your ability to work or study.

Activity Limitations

Flu can have a wide range of physical and mental effects on a person. From brain fog and mood changes to inflammation and a worsening of chronic conditions – a flu makes it hard for you to perform your usual work activities such as intensive research, attending meetings, or traveling. 

In this part of the Note, the doctor identifies the specific activity limitations that should apply in your case. In addition to two standard options – Strenuous physical activity, and Limited cognition due to fatigue, we have also added a third “Other’ category where you can specify any other type of activity you need to refrain from on account of your flu condition.

This section will give your supervisor or HR manager a better sense and understanding of why you can not be expected to productively perform in light of the nature of your job duties & health condition.

Return To Work

Flu can last anywhere from a couple of days to two weeks. That said, different symptoms may come and disappear at different rates. For instance, a fever may go away a lot more quickly than, say, a runny nose.

In this part, the doctor shall mention a projected date of recovery in light of your unique symptoms and individual medical history. If recovery is imminent, you can simply check the box next to Today or Tomorrow.

In case of a longer expected period of recovery, the doctor must identify a prospective date when the patient can be expected to resume his usual work duties or academic obligations.

Doctor’s Verification

The last part of the Note is all about ensuring that the information provided is attributed to a responsible medical professional.

Make sure your doctor puts his professional stamp here which should ideally include the physician’s name, specialty, and registration number.

Seal the deal with a dated signature from your doctor, verifying that the information included in the Note is true and accurate to the best of their knowledge and expertise.
