Strep throat is a common, easily spread infection of the throat and tonsils. Thanks to physical symptoms like high temperature, swollen lymph nodes, body aches, and even vomiting, this condition can make it difficult for you to discharge your normal duties at work.

So if you ever find yourself in the grip of a sore, strep throat, you will need a few days of rest to help you recover. But before you submit an official leave application to your employer, you might need a doctor to endorse a formal medical note explaining what the condition entails for you.

Doctor’s Information

Right at the top-left of the Note, will mention the name of your doctor, their educational or professional qualifications, as well as the postal address of the clinic or hospital where they currently practice. The idea here is to make sure that the information provided can be attributed to a responsible & registered medical professional.

You will also need to add the contact number of the clinic, thus ensuring your employer has a way to double-check and confirm any details recorded in the note.

The Note

Our Note provides a brief yet comprehensive overview of what strep throat is, and how it affects the human body. This is done in a way to show why the condition not only makes it difficult for you to remain physically and mentally active but also in terms of the impact it has on the well-being of people around you.

In this Note, we cover some basic information about the condition, summarised in bullet form below:

  • A formal diagnosis (Strep throat)
  • Cause (Bacterial infection caused by Streptococcus pyogenes)
  • Typical symptoms (coughing, swollen lymph nodes)
  • Ideal conditions for rest period (physical rest, and adequate hydration)
  • Limitations of activity (Strenuous physical activity, socialization)
  • Nature of Treatment (Antibiotics, supportive care)

In our template, we particularly try to underscore the contagious character of the infection (which primarily spreads through respiratory droplets) since this information is relevant and important for any HR professional or organization who wants to keep their staff and members safe from harm.

At the end, we add a short note of gratitude to the employer for supporting the patient’s recovery.

Seal the deal with a dated signature from your doctor, verifying that the information included in the Note is true and accurate to the best of their knowledge and expertise.

This is the most important part of our template as it specifies the duration (in terms of days) for which the patient should be on rest. The ideal resting period shall vary depending on the nature and severity of the symptoms you are experiencing. In the date placeholders,  your doctor can adjust this period depending on your individual health condition and the nature of your job.
