The “Organization Details” section captures essential information about your organization. This includes the full name of your organization, its address, and the place where the receipt is issued. Additionally, you write the Employer Identification Number (EIN) which is a unique identifier for the organization as per IRS standards. A link to the IRS website is added for verification. 

Next, in the “Donor Details” section write information about the donor. This includes their full name, contact number, address, and occupation. It also has fields for the organization name, if the donor is representing an organization, and the date and total amount of the donation.

The “Acknowledgement” section contains crucial statements for tax purposes. It includes a checkbox for the donor to acknowledge that they did not receive any cash, goods, or services in exchange for the donation, which is a requirement for the donation to be tax-deductible. Another statement confirms the nonprofit’s classification as a 501(c)(3) organization under IRS standards.

Finally, there is space for an authorized representative or donor’s signature and date. At the bottom, a thank you note has been added. You can edit this statement to include your organization’s name here.
