How to Use this Form

After this, you can note down the time and date of the assistance request as well as the name of the person who requested it. 

Owner’s Details

This section collects information about the vehicle owner, such as their name, phone number, city, state, and zip code.

Driver’s Details

This is for the driver’s information. It mirrors the owner’s details but refers to the person driving the vehicle, which may be different from the owner. Add the driver’s name, phone number, and address, including city, state, and zip code.

Vehicle Information

This part gathers the vehicle’s key identification, including the license number, make, model, and year. Input the vehicle’s license plate number, make (e.g., Ford), model (e.g., Focus), and year of manufacture.

Vehicle Towed To

You can specify the location where the vehicle is being towed in this section of the template.  Write the address or name of the facility where the vehicle will be towed. Since towing charges can vary greatly between urban and remote regions, it is wise share the fair with the cat owner and properly indicate that in your form to avoid any disputes over costs.

Reason for Service

This part allows you to select the reason the vehicle needs towing, such as an accident, abandoned vehicle, tow zone, or impoundment. You can select the appropriate box that describes the reason for the tow, like an accident or impound.

Mileage

This section calculates the distance the vehicle will be towed and the related charges. Enter the mileage before and after the tow to calculate the total miles. You can then include the rate charged per mile.

Labor Time

This section records the hours worked to complete the service. First, write the start and finish times, then calculate the total labor hours. Multiply by the hourly rate to get full labor costs. 

Special Equipment

If any special towing equipment was used, this section allows for selection (e.g., dolly, winch, snatch block). Check the equipment used during the tow, like dolly or winch, to ensure it’s recorded.

Extra Person(s)

If extra personnel were involved in providing roadside assistance, this section captures their details and any additional charges. Write the name and number of extra personnel in the provided field.

Type of Tow

Here, you specify the type of tow service, such as sling hoist tow, flatbed tow, or wheel lift. Check the correct box based on the tow type, like flatbed tow or wheel lift.

Towed Per Order Of

This specifies who requested the tow, such as the police, owner, or property manager. You can select the checkbox that matches who ordered the tow.

Damage Area(s)

If the vehicle was damaged, this section documents the affected areas. Note any visible damage to the vehicle’s exterior or interior areas before tow and write it down. 

You might be responsible for covering the cost of the damage if the car’s owner claims that the tow caused the damage to their vehicle. Therefore, it is wise to thoroughly inspect the vehicle and document any existing damages before towing. 

Storage Charges

If the vehicle will be stored after the tow, this section records the daily rate and total storage cost. In many states, there are regulations on what can be charged for storing vehicles after towing. Enter the daily storage rate and the total number of days, and then calculate the total charge.

Payment Method

Write how the customer will pay, whether by cash, check, or card, in this section of the template. Select the relevant payment method and add a card or check details if applicable.

Towing Charges Summary

This is a breakdown of all costs, including towing, mileage, labor, storage, and additional charges. You can sum up all charges, including towing, mileage, and any extras, and then calculate the subtotal and add tax for the grand total.

In most states, there are strict regulations about how much you can charge for a towing service. It is important to provide a clear, itemized summary of all charges to maintain transparency regarding the costs. 

Signatures

This part includes fields for the signatures of the tow operator, the person releasing the vehicle, and the authorized person receiving it. The tow operator, vehicle releaser, and authorized recipient should each sign and date in the appropriate fields.
