If the child is traveling alone, both parents should write their names in the text field; if the child is traveling with only one parent, the other parent should write their full legal name.

Start with mentioning the name of the destination where the minor would be traveling. It can be the name of the city or state. If the trip is international, then write the name of the city and the country here. 

Then in the next fields, write the exact dates of travel. After this, mention the reason for travel, i.e., family visits, school trips, sports club trips, etc.

After this, you can sign the form in the designated text fields. Besides parents, text fields are also provided for the signatures of the witness and any accompanying adult, if applicable. Who is a Minor Child?

The definition of a minor child varies across states in the US. In 47 of the 50 states, someone is considered under the age of majority if they are under 18 years of age. In Alabama and Nebraska, this age is 19 and in Mississippi, the age of majority is considered to be 21. 

You would still need the other parent to sign a letter granting permission for the child to travel with you, even if you have full custody of the child. This is so because, in most cases, the other parent has visitation rights, and they will be affected when the child travels.

Is My Child Allowed to Fly Alone?

There are instances where children might be flying alone, and there are no Department of Transportation regulations regarding that. However, most airlines follow a standard procedure when children are not accompanied by an adult. On most flights, children under the age of 5 cannot travel alone.

For domestic flights, children from the age of 5 to 7 can travel alone on direct flights. However, children from 8 to 11 can travel alone on any kind of domestic flight with parental approval. For international flights, children above 12 and up to 17 are allowed to travel on their own, but permission from their parents is required in most cases.
