But before you call in sick, chances are, you’ll need a formal medical note as proof of your debilitated physical or mental health, otherwise called a doctor’s note.

A doctor’s note, whether in the form of a medical certificate, an excuse slip, or a release form, is a verifiable document proving an individual has been professionally attended to by a registered doctor. It includes key details relevant to each case, including a formal medical diagnosis, a treatment plan for the future as well as any necessary restrictions or adjustments needed in light of the patient’s condition.

When Do You Need a Doctor’s Note?

Now, let’s be clear: a doctor’s note is for more than just securing a leave from work or skipping school on medical grounds. It can also be used to prove that someone is physically and mentally fit to work after childbirth, a major surgery, or an accidental injury. Moreover, in some industries or job roles, you might need to demonstrate a certain level of physical fitness to perform a specific physical activity associated with that role. Pilots, for example, must have their vision tested, and military personnel must meet strict physical requirements before being inducted.

You also need a doctor’s note if you want to be excused from jury duty on account of being sick. Similarly, your insurance company might request you for one if you wish to cancel a pre-booked trip or flight due to a medical issue.

What We Offer?

At WordLayout, our team conducted in-depth research into some of the most commonly sought-after medical notes out there. Based on this exercise, we developed a series of customizable templates to suit a variety of different medical needs and professional contexts.

Doctor Notes, Law, and Patient Rights

To safeguard patient autonomy and uphold the integrity of the medical profession, all medical records must comply with state and federal laws. Here in the U.S., any note issued by a registered healthcare provider for third parties (such as employers or college administration) must abide by certain legal and ethical considerations.

Firstly, doctors have a general duty in tort to be truthful when verifying patient information; falsifying details can have legal consequences for them and of course, the patient. Moreover, they must comply with specific patient privacy laws such as the Health Insurance Portability and Accountability Act; for example, only sharing personal information with the patient’s knowledge who must authorize this in the form of specific written permission. Doctors must limit information to only necessary information, and ensure no unauthorized person sees it.

Note: Even though doctors are under no universal legal obligation to issue doctor notes (although most still do to ensure patient’s welfare) – in some cases, they might have to. Let’s say, you are an occupational physician particularly employed to assess someone’s fitness for work, in such a case – refusing to issue without a legitimate reason would probably break your contract.

A 6-Step Guide For Using Our Templates

Using a template can help you produce consistent and efficient documentation for all your patients that is up to par with professional standards. Here’s a step-by-step guide to help you make the most of our medical note templates:

  • Select the Right Template: Start by choosing the appropriate doctor’s note template from the collection (say, for flu, allergies, a mental health condition, etc.). Now think about whether you need a version with a letterhead or without. 
  • Download the Template: Choose the file format that suits your needs: Google Docs, Microsoft Word, or PDF. Download and save it to your device.
  • Fill Out the Template: Open the template in your chosen software and fill in the necessary details. Be sure to include all required information.
  • Get It Signed and Stamped: If the note requires a signature or stamp, make sure these are added. This step might be done digitally or by printing and signing by hand.
  • Print the Note: If you’re using the version without a letterhead, print it on your own letterhead. Otherwise, print the template as it is.
  • Personalize: If needed, doctors can add any additional information to tailor the note to the specific patient.
