  • The organizational make-up of every individual company is different. For instance, if you are a small start-up, you might not have separate Departments, and the Head of Department might often be the Chief Executive Office (CEO) themself. Employees should be advised to confirm these details with the company’s senior management when filling out a Check Request Form. 
  • For each and every item of payment, the organization must ensure that the person filling out the form attaches necessary documents of proof such as original invoices, tickets, bills, receipts or e-records of online transactions. Failure to attach these can result in a form being deemed invalid, and not being approved for further processing.
  • Different companies also have different policies when it comes to what is a justified expense. For example, a health-conscious company might provide a wellness stipend for employees to join a local gym or a yoga class, while other companies might offer no such benefit. For this reason, it is important for organizations to make sure that the funds being requested only cover those expense categories that are part of the company’s policies.

The Check Request Form Process

Processing a Check Request involves a number of stages, from the point when a request is initiated to the point when the funds are disbursed. This process requires the input of different individuals or departments working within an organization, such as:

  1. Department Managers or Supervisors: They may submit check requests for department-related expenses or approve requests submitted by their team members before forwarding them to finance.
  2. Finance/Accounting Department: This department processes the check request forms. They review the form, verify supporting documentation, and issue the payment in the form of a check.
  3. Administrative Staff: Administrative personnel may handle check requests on behalf of others, particularly in large organizations where they manage payments for departments or executives.
  4. Senior Management: Senior management in a company (or in case of a university, the Dean or a Head of Department) might need to approve larger payments or requests that exceed a certain threshold.

Here are 5 tips very organization should encourage requestors to follow when filling out a Check Request Form:

  • Make sure they complete the entire form, and fill out all required fields.
  • In the case of ID numbers such as Employee ID, Student ID, or SSN/TIN, they should always be advised to double-check to make sure they get the digits or alphabets (and their order) right.
  • Make sure they don’t forget to attach supporting documentation, such as invoices or receipts for each expense (for example, if the reimbursement is travel-related, the original ticket or a copy of the conference/event schedule may be included)
  • If there is not enough space on a form to explain the reason for the request or the details of an expense, they should be encouraged to include a separate document and attach it to the form.
